Recipient Resources

This page provides information and resources to help grant recipients manage their awards effectively, from executing a grant agreement to successfully implementing a project. You will find key guidance manuals, links to forms and templates used in the grant program, and new awardee/grant recipient workshop materials.

Please be sure to reference the materials specific to your awarded project’s grant round, as requirements may differ from those in other years.

General Forms/Templates

Blank forms used in the AMMP are linked below and are also listed on CDFA's forms page. Grantees will receive project-specific copies of required forms at the beginning of the grant term or upon request for forms that are used only as needed. Grantees can always request copies of these by reaching out to cdfa.oefi_ammp_tech@cdfa.ca.gov.

STD 204 Payee Data Record Examples

After being notified of a grant award, grantees will receive instructions to complete a Payee Data Record form. This is an important first step and required before a grant agreement can be established for the awarded project. Find examples of completing this based on entity type.
